1. Dental Plus Plan - $19.95 per month for an entire household. This plan allows you to save up to 80% on all dental services including braces, cosmetic dentistry, and dental implants. In addition to dental services, this plan allows members to save money on prescriptions, chiropractic treatment and vision care including Lasik eye surgery. There are no deductibles, no waiting periods, and all ongoing conditions are accepted.
2. Basic Health - $29.95 per month for an entire household. Includes savings on physician visits, ancillary services, hospital advocacy and a 24/7 nurseline to answer any medical questions that you may have. This plan also provides savings on diabetic supplies, hearing aids, mental health services, lab services, imaging studies, physical therapy and health screening.
3. Total Health - $39.95 per household per month. This combines all of the services in the Dental Plus and Basic Health programs. The plan includes Dental, Vision, Prescription, Chiropractic, Physician visits, Ancillary Services, Hospital Advocacy, 24/7 nurseline, and additional services.
4. Total Health Plus - $59.95 a month per household. This is the complete package. This includes everything in the Total Health package plus the AmeriPlan Auto Club, AmeriPlan Identity Theft Assistance, and AmeriPlan Legal.
